Resumen

OBJETIVO: valorar los resultados, con tres fármacos diferentes, del tratamiento agudo a 48 horas y a 7 días del parto pretérmino, cuál de ellos puede aportar mejores resultados con menos complicaciones.

MATERIAL Y MÉTODO: estudio longitudinal y prospectivo efectuado en tres grupos de pacientes entre 24 y 34 semanas de embarazo. El primer grupo se trató con indometacina, el segundo con salbutamol y el tercero con atosibán. Se efectuó el análisis estadístico entre los tres grupos para valorar éxito, fracaso, ambos a 48 horas, inhibición de la contractilidad a 7 días y efectos colaterales durante la inhibición en fase aguda. Todas las pacientes de los tres grupos recibieron un esquema de betametasona para inducción de madurez pulmonar.

RESULTADOS: el grupo de indometacina lo integraron 656 pacientes. Hubo éxito en la uteroinhibición a 48 horas en 582 pacientes, fracaso en 74, inhibición a 7 días en 429 y efectos colaterales en 174 pacientes. El grupo de salbutamol se integró con 776 pacientes: 658 alcanzaron la uteroinhibición a las 48 horas, se registró fracaso en 118, e inhibición a 7 días en 564 y efectos colaterales en 302. El grupo de atosibán se conformó con 82 pacientes, en 75 se consiguió la uteroinhibición, en 7 fracaso y en 69 inhibición a los 7 días y efectos colaterales en 4. 

CONCLUSIONES: los resultados de la uteroinhibición en fase aguda fueron similares entre los tres fármacos, no así la uteroinhibición a 7 días y en efectos colaterales en las que se observó mejor diferencia en el grupo tratado con atosibán (p= 0.001).

Abstract

OBJECTIVE: To asses the results of three different drugs in the treatment of acute premature labor within the first 48 hours and after 7 days of the event, in order to determine which of them provides better results with the least number of complications.

METHODS: Longitudinal, prospective study in three groups of patients between 24 to 34 weeks of pregnancy. The first group, with indomethacin; the second group, with salbutamol, and the third group with atosiban. Statistical analysis of the three groups to compare success/failure rates within the first 48 hours, inhibition of uterine contractility after seven days, and side effects during the accute stage of uterine inhibition. The same data was analyzed in all three groups. All patients in the three groups received a betamethsone régimen to induce fetal lung maturity. 

RESULTS: The indomethacine group, with 656 patients, showed uterine inhibition success within 48 hours in 582 patients, failure in 74 patients, uterine inhibition after seven days in 429 patients, and side effects in 174 patients. The salbutamol group, with 776 pacients, showed uterine inhibition success within 48 hours in 658 patients, failure in 118 patients, uterine inhibition after seven days in 564 patients, and side effects in 302 patients. The atosiban group, with 82 pacients, showed uterine inhibition success within 48 hours in 75 patients, failure in 7 patients, uterine inhibition after seven days in 69 patients, and side effects in 4 patients.

CONCLUSIONS: The results for accute fase inhibition of uterine contractility were similar for the three drugs, while uterine contractility inhibition after seven days and side effects showed an improvement in the group treated with atosiban (p=0.001).
