Resumen
OBJETIVO: comparar las diferencias en el resultado de la histerectomía total laparoscópica, indicada por una alteración uterina benigna, en úteros de más o menos de 500 g.
MATERIAL Y MÉTODO: estudio de cohorte, comparativo, retrolectivo efectuado en el Instituto Nacional de Perinatología de enero de 2009 a junio de 2015. Los datos se obtuvieron de los expedientes de pacientes a quienes se practicó histerectomía total laparoscópica. Se registraron las características generales del procedimiento y las complicaciones. Para la comparación de las variables se utilizó la prueba t de Student, U de Mann-Withney, χ2 o prueba exacta de Fisher según el tipo de variable y distribución. Se consideró estadísticamente significativo el valor de p<0.05 .
RESULTADOS: se registraron 635 pacientes: 24 en el grupo A (útero igual o mayor de 500 g) y 611 en el B (útero menor de 500 g). Se encontraron diferencias estadísticamente significativas en el peso uterino (grupo A: 678 ± 132 g y grupo B: 180 ± 93 g; p=0.001), tiempo quirúrgico (grupo A: 190 ± 90 min; grupo B: 151 ± 46 min; p=0.001) y pérdida sanguínea (grupo A: 302 ± 188 mL; grupo B: 199 ± 43 mL; p=0.04). La mayoría de las pacientes del grupo A requirió morcelación de la pieza quirúrgica (80.8 vs 6.9%, p=0.01). El resto de las variables fueron similares entre los grupos.
CONCLUSIONES: la histerectomía total laparoscópica es una buena opción de tratamiento quirúrgico en pacientes con útero de gran tamaño (≥ 500 g).

Abstract
OBJECTIVE: To compare the outcomes of two groups of patients in which a total laparoscopic hysterectomy were performed: group A were the patients with a uterine weight more or less than 500 g, and group B patients with a uterine weight less than 500 g.
MATERIALS AND METHOD: Comparative and retrospective study was conducted at the Instituto Nacional de Perinatología from January 2009 to June 2015. We obtained the files of the patients with a total laparoscopic hysterectomy. Also registered general data, surgical procedure and postsurgical complications. The variables were compared using Student's t test, Mann-Withney U test, χ2 and Fisher exact test, according to the variable type and its distribution. We considered a p value < 0.05 as statistically significant.
RESULTS: We included 635 patient, 24 into the group A and 611 patients into the group B. We found statistically significant differences in the uterine weight (group A: 678 ± 132 g; group B: 180 ± 93 g; p=0.001), surgical time (group A: 190 ± 90 min; group B: 151 ± 46 min; p=0.001), and amount of blood loss (group A: 302 ± 188 mL; group B: 199 43 mL; p=0.04). Morcelation was more frequently used into the group A for the extraction of the surgical piece (80.8 vs 6.9%, p= 0.01). The rest of comparisons between groups were very similar.
CONCLUSIONS: Total laparoscopic hysterectomy is a good option for the surgical management of large uterus.
